Piano Playing and Cognitive Aging in Adults
Every few months I get a message from an adult who mentions, almost as a side note, that their doctor suggested picking up something mentally challenging. Sometimes a spouse is starting to worry. Sometimes it is the adult themselves, quietly keeping tabs on their own sharpness. They want to know whether piano lessons could actually help their brain, or whether that idea is more hope than science.
I think they deserve a straight answer. So I spent some time with the research, and here is what I found: the news is genuinely encouraging, but it is not as simple as the headlines sometimes make it sound. Let me walk you through what the studies actually say.
What the Research Shows—and What It Doesn't
There are two kinds of evidence in this area, and they are not equal. The first kind is observational: researchers look at large groups of older adults, ask who plays instruments and who doesn't, and then compare cognitive test scores. These studies consistently show that musically active adults tend to score better on measures of memory, attention, and executive function. A 2022 paper in Frontiers in Psychology found associations between lifetime musical activity and better cognitive and brain outcomes across multiple domains. A 2024 cohort study found that older adults who played instruments showed better working memory and executive function than those who didn't.
Those findings matter. But they cannot tell us that music caused the difference. People who pursue music throughout their lives may also read more, stay socially connected, exercise regularly, or have had more years of formal education. Any one of those things—or all of them together—could explain part of what we're seeing. Researchers call this the confounding problem, and the honest ones acknowledge it plainly.
A 2023 review article, drawing on data from a very large study of more than 52,000 older adults, reported lower dementia risk in musically engaged groups. But the same review flagged that the evidence is limited and that reverse causality is a real concern. That means it's possible that people who are already cognitively sharper simply stay musical longer, rather than music being what kept them sharp. We can't easily tell from that kind of data.
The second kind of evidence—randomized controlled trials—is harder to do but more telling. Here, researchers actually assign some older adults to piano instruction and others to a control condition, then measure what changes. The trial evidence is modest in size, and not every study agrees, but it points in an interesting direction.
The Trials Worth Knowing About
The study I find myself thinking about most often is a 2007 randomized trial published in Aging, Neuropsychology and Cognition. Thirty-one adults between sixty and eighty-five were divided into a piano instruction group and a control group for six months, with a three-month follow-up period afterward. The piano group showed better performance on two well-established cognitive tests—the Trail Making Test and the Digit Symbol test—than the control group. Those tests measure processing speed, attention, and the ability to shift between mental tasks. Small study, yes. But it was randomized, which gives it more weight than a survey asking who plays piano.
A later line of research, with a trial protocol registered in 2015 and results published in 2022, compared piano training, computer-assisted cognitive training, and a control condition in adults sixty and older. Both the piano group and the computer training group improved on working memory and processing speed. Piano training had one additional benefit that the computer training did not: it helped specifically with category switching verbal fluency, which is the ability to mentally shift between different groups of ideas. That kind of flexible thinking is an executive function, and it is exactly the kind of skill that tends to slip with age.
Then there is a 2023 trial published in The Journals of Gerontology that compared piano training directly with music listening. This one found no significant benefits for auditory processing, cognition, or everyday function in the piano group compared to the listening group. I want to be clear about that result, because it matters. It does not mean piano is useless—but it does mean we cannot assume that any form of musical engagement with a keyboard automatically produces measurable cognitive gains. The picture is more complicated than that.
Where the Evidence Is Strongest
Putting all of this together, here is my honest read:
- Processing speed, working memory, and task switching are the areas where the trial evidence is most consistent. Two separate randomized studies found improvements in these domains.
- General memory benefits are less clear. Some reviews mention them, but the individual piano trials don't all agree.
- Dementia prevention is the claim I'm most careful about. The observational data are intriguing, but claiming that piano lessons prevent dementia goes well beyond what the current evidence can support.
One practical frustration I noticed in the research: almost none of the studies clearly reported how much the participants were actually practicing. Did they play twenty minutes a day? An hour? Three times a week? We don't know from the snippets available, and that matters enormously for anyone trying to figure out what a realistic commitment looks like. Observational studies typically recorded only whether someone played an instrument at all, not how much. That gap makes it difficult to give a confident answer to the very reasonable question: how much is enough?
What This Means If You Are an Adult Learner
If you are an adult who is considering lessons, or who is already taking them and wondering whether this is time well spent, I want you to hear this clearly: the evidence does not prove that piano playing prevents dementia. Anyone who tells you otherwise is overstating what science has shown so far.
What the evidence does suggest is that learning and practicing music is a genuinely demanding mental activity—one that appears to challenge processing speed, working memory, and cognitive flexibility in ways that may be meaningful. The brain is doing real work when you read a new piece of music, coordinate two hands independently, keep a steady tempo, and adjust your touch all at once. That is not a trivial workout.
I also think there is something the studies can't fully capture. Adults who come to my studio often talk about the satisfaction of making something beautiful, the focus required to really listen to themselves play, the way an hour at the piano feels different from an hour scrolling a screen. Whether or not a future trial confirms those benefits in a statistically significant way, they are real to the people experiencing them.
If you already have health concerns about cognitive aging, piano lessons are not a substitute for medical care. But they are a rich, engaging, skill-building activity that a growing body of research views favorably—with appropriate caution about the limits of what we know.

What we can say with confidence is that learning to play piano is a demanding, rewarding mental activity, and for adults who stick with it, the brain is genuinely working hard every time they sit down to practice.
